Patient Forms
After you call our office and schedule your first appointment with us, you will receive a text with a link to fill out our new patient forms. If you lose the link or would prefer to download them at home instead, we have also included a copy of them below. Simply click on the link to access them. Pay Online
For patients with outstanding balances, you can easily take care of it by submitting a payment through our secure, easy-to-use payment portal linked below. If you would prefer, you can also call our front desk and submit a payment over the phone.
Dental Insurance
At Nobles Dental, we’re happy to accept most PPO dental insurances as well as CHIP and children’s Medicaid. Our team will file claims on your behalf to help you maximize your benefits and minimize any out-of-pocket costs. We’re also happy to help you navigate your plan, as we understand dental insurance can sometimes be complicated to figure out on your own.
In-House Dental Membership Plan
Are you uninsured? Our Nobles Dental in-house membership plan offers a simple alternative to dental insurance. By paying a flat monthly or annual fee, you can receive coverage for all of your basic preventive visits, like your checkups and cleanings, routine X-rays, and discounts on additional services you may require throughout the year. CareCredit & Financing
If you’re receiving a treatment that involves an out-of-pocket cost or isn’t covered by your insurance, we also offer additional financing to make paying for it as easy as possible. We work with both CareCredit and Sunbit to offer our patients low monthly payment plans at little-to-no interest.
